Certificate Request Form / 証明書交付願

The Yushima International Student Support Group issues the following certificates (in Japanese and English):

・Japanese Government (MEXT) Scholarship Certificate
・HONORS Scholarship Certificate

Please download and fill in "Certificate Request Form", and then send the completed form to the Yushima International Student Support Group via email.

【Submission】 Yushima International Student Support Group: fssu@ml.tmd.ac.jp * In general, it takes about one week - 10 business days to issue a certificate. * Please contact the Yushima Student Office regarding academic transcripts and enrollment certificates.

湯島留学生支援グループにおいて、下記の証明書（日英）を発行いたします。

・国費受給証明書
・学習奨励費受給証明書

「証明書交付願」をダウンロードして必要事項を記入し、ファイルをメールで湯島留学生支援グループに提出してください。

【提出先】湯島留学生支援グループ fssu@ml.tmd.ac.jp ※証明書の発行には、通常1週間から10日程度かかりますのでご了承ください。 ※成績証明書、在学証明書については、教務課湯島教務室にお問い合わせください。
