１．Registration for the First Time

     Mon.-Thur. A.M.9:00

２．We ask all patients visiting for the first time to bring a referral (patient referral document) from amedical doctor ( not a dentist) in another medical hospital or clinic. If you have glaucoma, please bringa referral from Eye doctor.

３．Please bring your medication notebook (okusuri techo).
